Please print form and fill out all fields legibly. Also, be sure to list all names used as well as the city and state of previous residency for the last 10 years. Once the form is signed and dated it can be scanned and e-mailed to orionint@mt.net or faxed to our secure fax at # (406) 458-8787. Please insert name of individual being screened along with the date of submission in the supplied fields near the top of the page. Also supply your contact information in the upper right hand portion. Simply check next to the type(s) of investigation(s) you wish to perform on the applicant. Whenever possible please submit the Request for Investigation with the completed Authorization for Release of Information form as the investigation cannot begin until Orion has received both forms. The form can be submitted by email to orionint@mt.net or to our secure fax at # (406) 458-8787.
